    • Predictive value of tenascin-C in coronary heart disease and coronary atherosclerosis severity

      Abstract:Aim To evaluate the predictive value of tenascin-C (TNC) in coronary heart disease (CHD) and coronary atherosclerosis severity. Methods 160 patients with chest pain who had undergone coronary angiography were included. According to the results of the angiography, the patients were divided into two groups:CHD group (n=90) and non-CHD group (n=70). The coronary atherosclerosis severity was evaluated by Gensini score, and the CHD patients were divided into three groups:low Gensini score group (n=39), medium Gensini score group (n=27), high Gensini score group (n=24). The levels of serum TNC were compared in each group. Pearson correlation analysis and multivariate linear regression analysis were used to evaluate the correlation between TNC and Gensini score. The predictive value of TNC in patients with coronary heart disease and high Gensini score was analyzed by ROC curve. ResultsThe level of serum TNC in CHD group was significantly higher than that in non-CHD group (P<0.001), the serum TNC level of high Gensini score group was significantly higher than that of medium Gensini score group, and the serum TNC level of medium Gensini score group was significantly higher than that of low Gensini score group (P<0.001). Pearson correlation analysis and multivariate linear regression analysis showed that there was a significant positive correlation between the serum TNC level and Gensini score in CHD group (P<0.001). ROC curve analysis showed that the area under curve (AUC) of TNC for predicting CHD was 0.811 (95%CI:0.768-0.974), and the Cut-off value was 11.94 μg/L, the sensitivity and specificity were 83.5% and 76.4% respectively. The AUC of TNC in predicting patients with high Gensini score was 0.944 (95%CI:0.816-0.989), and the Cut-off value was 16.47 μg/L, the sensitivity and specificity were 82.6% and 78.4% respectively. Conclusion Serum TNC level is significantly correlated with Gensini score of CHD patients, which has a certain predictive value for CHD and coronary atherosclerosis severity.

      Abstract:Coronary artery endothelial injury is the initiating and the most critical factor of atherogenesis. For a long time, researchers have focused on the mechanism of endothelial injury induced by oxidized low density lipoprotein(ox-LDL). With the development of new technologies and methods, the role of micro-RNA (miRNA) in the process of endothelial cell injury induced by ox-LDL has been gradually discovered. This review briefly describes the miRNAs involved in endothelial injury such as endothelial inflammation, autophagy, apoptosis and dysfunction under the condition of hyperlipidemia. Summarizing these miRNAs may permit new ideas for the prediction, diagnosis and even treatment of coronary atherosclerosis.

      Abstract:Aim By examining the level of CD45 protein expression in coronary atherosclerotic lesions, and to analyze the relationship between CD45 and lesions structure changes and explore the role of CD45 in the development of coronary atherosclerotic lesions in humans. Methods Specimens of coronary arteries were obtained from Forensic Judicial Expertise Center, Guizhou Medcial University with coronary atherosclerosis. The coronary arteries were classified into three groups according to the lesions:control, atherosclerosis, atherosclerosis and secondary lesion. Routine HE staining of paraffin sections was performed to observe the histological structure of coronary artery in the three groups. The expression level of CD45 protein was detected by immunohistochemical staining, Western blot and Real-time PCR. The relationship between the expression of CD45 and the structural changes of atherosclerotic lesions were analyzed. Results (1)Compared with the control group, the thickness of the fibrous cap in the coronary arterial plaque with atherosclerosis and secondary lesions became thinner, and the thickness of the lesion, the thickness of the necrosis lesion, and the lumen area of the vessel increased (P<0.05). (2)There was a statistically significant difference in the expression of CD45 protein between vascular tissues. (3)There was a statistically significant difference in the expression of CD45 between the control group, the atherosclerosis group and the secondary lesions, and it was mainly expressed in the all leukocytes of the plaques shoulder and bottom. (4)There was a statistically significant difference in the expression of CD45 mRNA between three groups. (5) CD45 expression was related to lesion structure changes. Conclusion The level of CD45 expression reflects the extent of inflammation in human coronary atherosclerotic plaque, and the extent of inflammation within the lesion can affect the structure of the lesion.

    • Correlation of non-high density lipoprotein cholesterol levels and coronary artery atherosclerosis severity

      Abstract:Aim To study the relationship between non-high density lipoprotein cholesterol levels and severity of coronary artery atherosclerosis. Methods This research studied 843 consecutive subjects undergoing coronary angiography who did not receive any lipid-lowering therapy. The severity of coronary artery atherosclerosis was determined using the Gensini score (GS)system. The patients were classified into the four groups according to the quartile of the GS. Then the patients were divided into three groups:the sigle-vessel group, the double-vessel group, the triple-vessel group.Lipid parameters were measured in all groups. The correlation analysis, receiver-operator characteristic and logistic regression analysis were performed, respectively. Results LDLC, non-HDLC and TC/HDLC were all dramatically increased according to the quartiles of GS (P<0.05, respectively), TG/HDLC in upper GS quartile was higher than the 1st and 2nd GS quartile. LDLC, non-HDLC, TC/HDLC in the double-vessel group, the triple-vessel group were significantly higher than those of the sigle-vessel group(P<0.05, respectively), TG/HDLC level had no significant difference. non-HDLC(r=0.315,P<0.01) was more closely related to GS than LDLC(r=0.252,P<0.01), TC/HDLC(r=0.242,P<0.01), TG/HDLC(r=0.123,P<0.01). The area under receiver operating characteristic curve(AUC) of non-HDLC for predicting high GS was 0.729(95%Cl 0.691~0.767,P<0.01) higher than LDLCs 0.694(95%Cl 0.653~0.734,P<0.01), TC/HDLCs 0.681(95%Cl 0.640~0.721,P<0.01), TG/HDLCs 0.564(95%Cl 0.521~0.607,P<0.01). Multivariate logistic regression analysis suggested that non-HDLC(OR=2.2,5%Cl 1.962~3.115) was slightly superior to LDLC(OR=2.5,5%Cl 1.785~2.875), TC/HDLC(OR=1.6,5%Cl 1.483~2.175), TG/HDLC(OR=1.022, 95%Cl 0.927~1.127) in predicting high GS after adjusting for potential confounders. Conclusion non-HDLC levels is a better predictor for the coronary atherosclerosis severity compared with LDLC, TC/HDLC, TG/HDLC.

    • Relationship Between Retinopathy and Coronary Atherosclerosis Determined by Coronary 64-Slice Multidetector Computed Tomography Angiography in Type 2 Diabetes

      Abstract:Aim To evaluate the relationship between diabetic retinopathy (DR) and subclinical coronary atherosclerosis (CAs) on coronary 64-slice multidetector computed tomography angiography (MDCT) in individuals with type 2 diabetes mellitus. Methods From July 2007 to December 2009, 114 and 124 type 2 diabetic patients with and without CAs were enrolled. They received fundus photochromy, coronary 64-slice multidetector computed tomography angiography, physical examination, and measurement of fasting plasma glucose(FPG), glycosylated haemoglobin (HbA1c), plasma lipid profile, estimated glomerular filtration rate (eGFR) and urinary albumin excretion rate(UAER). Then the analysis of the relationship among the detection and measurement outcomes were conducted. Results Diabetic patients with CAs had a higher prevalence of DR than those without CAs (67.5% vs 33.1%, P<0.001). After adjustment for the traditional risk factors for cardiovascular disease, CAs was independently associated with DR (OR5.0, 95%CI 2.6~9.8). There was significant difference in the prevalence of CAs by the number of CAs vessels among patients without DR (NDR), those with pre-proliferative retinopathy (pre-PDR) and those with proliferative retinopathy (PDR) (P<0.01). The prevalences for CAs, the prevalences of CAs ≥3 vessels involved by plaque, and proportions of vessels with significant coronary plaque and of involved vessels in all detected coronary arteries were significantly increased with the presence and severity of DR (NDR vs Pre-PDR, Pre-PDR vs PDR, P<0.01 for each). Conclusions The severity and extent of CAs were significantly increased with the incidence and progression of DR, and much of CAs and DR could be still multifactoral with common pathway.

      Abstract:Aim To observe the effect of Shenshao oral lotion to blood lipid,hemorrheology,and stent restenosis in patients after percutaneous coronary coronary intervention (PCI). Methods 1 023 patients with coronary heart disease were recruited into our study. Patients were randomly divided into two groups: conventional therapy group and ShenShao oral lotion taking group. Results The level of LDLC in patients of Shenshao oral lotion group decreased lower than that in conventional group in the same period. The indices of blood rheology in patients of Shenshao oral lotion were decreased lower than that in conventional therapy group. The late stage inner loss of the ShenShao oral lotion group was less than the conventional medicine group (P<0.05). Conclusion Shenshao oral lotion could effectively prevent restenosis after PCI.

    • Correlation Between Plasma Visfatin and Coronary Atherosclerosis in Xinjiang Kazak and Han Patients

      Abstract:Aim To expore the correlation between plasma visfatin and coronary atherosclerosis in Kazak and Han patients with coronary heart disease.Methods Forty Kazak and forty-five Han patients with coronary heart disease were picked out randmly as test groups aging from forty to seventy-five, then both test groups were divided into two subgroups (mild sub-group and heavy subgroup) using Gensini score respectively. Thirty-five Kazak and forty Han healthy people were picked out as control groups. Plasma visfatin was measured using enzyme-linked immunosorbent assay (ELISA), meanwhile liver function, renal function, blood lipids, blood glucose and fructosamine were measured.Results Kazak and Han patients with coronary heart disease had higher levels of plasma visfatin than control groups (P<0.01) the heavy subgroup had higher levels of plasma visfatin than the mild subgroup in Kazak and Han patients with coronary heart disease there was a positive correlation between the level of plasma visfatin and Gensini score in the two nationalities ( r0.925 and P<0.01 for Kazak; r0.967 and P<0.01 for Han).Conclusions Kazak and Han patients with coronary heart disease had higher levels of visfatin than their control group. There was a positive correlation between the level of plasma visfatin and Gensini score in the two nationalities, indicating that visfatin might be involved in the occurrence and development of coronary atherosclerosis.

      Abstract:Aim To explore the relationship of serum level of eosinophilic cation protein(ECP)and classification of coronary artery lesions through measuring their levels.Methods The study enrolls 70 patients with coronary heart disease(CHD),and 30 patients without CHD as controls.All patients were confirmed by angiography.According to the number of diseased vessels,we divided the patients into three groups: 1-vessel disease group,2-vessel disease group and 3-vessel disease group;and then analyzed the relationship between the level of ECP and the number of diseased vessels respectively.Results The serum levels of ECP in CHD group were higher than those in control group(P<0.01).The relationship between the serum levels of ECP and the number of diseased vessels were statistically related among three groups(P<0.01).Multiple linear regression analysis showed that levels of ECP were closely associated with the severity of coronary artery lesions.In multivariate analysis,ECP levels were significantly associated with CHD independent of traditional risk factors.Conclusions The study indicates that the higher serum levels of ECP may be related to the initiation and development of coronary artery lesions.So the higher serum levels of ECP is likely one of the indexes to predict the severity of the coronary artery lesions.

      Abstract:Aim To know about the incidence rate of carotid atherosclerosis of the patients with coronary heart disease;To analyze the related risk factors of carotid atherosclerosis of the patients with coronary heart disease;To determine the relationship between the aggravation of carotid atherosclerosis and coronary atherosclerosis;To summarize the pathologic character of carotid atherosclerosis of the patients with various coronary heart disease.Methods Review the result of coronary angiography and carotid ultrasound and the related risk factors of the patients who were diagnosed as coronary heart disease through coronary angiography.The patients were divided into the carotidatherosclerosis group and the non-carotidatherosclerosis group.The patients were divided into stable angina pectoris(AP) group,unstable angina pectoris(UAP) group and acute myocardial infaction(AMI) group according to the diagnotic criterion of coronary heart disease of WHO.The patients were divided into group A(coronary artery of single vessel lession),group B(coronary artery of double vessel lession),group C(coronary artery of triple vessel lession) and group D(left main vessel lession).The aggravation of carotid atherosclerosis was graded.Results There were 198 patients with various aggravative carotid atherosclerosis among 228 patients with coronary heart disease(86.8%).Carotid atherosclerosis was much related with hypertension and non-related with age,sex,body mass index(BMI),smoking,drinking,hyperlipidermia,hyperuricermia and diabetes mellitus.The aggravation of carotid atherosclerosis got graver with the severity of coronary athero-sclerosis.But only the plaque number of group A was more than group D(P<0.05).The grading integral and Crouse integral of carotid atherosclerosis of AP group was not remarkablely less than UAP group.The grading integral of carotid atherosclerosis of AP group was less than AMI group and the Crouse integral of AP group was more.But there was no remarkable difference.The number of all plaque,and plaque of AP group was not remarkablely less than UAP group and AMI group.The plaque of all the three groups had no obvious difference.There was the plaque among the three groups.Conclusion Carotid atherosclerosis is relative to coronary heart disease.

      Abstract:Aim To investigate the relation between the polymorphisms of angiotensinogen(AGT) genetpye M235T and angiotensin-converting enzyme(ACE) genetpye I/D and coronary atherosclerosis. Methods The study population was consisted of 151 patients with coronary heart disease(CHD),the healthy control group was consisted of 127 without CHD.Patients with CHD were compared with those persons in the healthy control group.AGT gene polymorphisms was tested with polymerase chain reaction and restriction fragment length polymorphism(PCR-RFLP) technology and ACE gene polymorphisms was tested with PCR. Results The ratio of AGT-TT and ACE-DD genetype in CHD group was separately 76.26% and 35.10%,which was significantly higher than the control group(44.10% and 14.96%).There was significant difference of the AGT-TT genetype between the CHD group and the control group P><0.01;so was ACE-DD genetype between the two group(P><0.01). Conclusion In the population of Chinese miao nationality,the gene polymorphisms of genetype AGT-TT and genetype ACE-DD are two risk factors of CHD morbidity and they are both independent of each other but also mutually coordinated.
