    摘要:

    目的 探讨加减枳实薤白桂枝汤(枳实、薤白、三七、人参等)对缺血再灌注大鼠心肌组织水通道蛋白(AQP)表达的影响及其可能的作用机制。 方法 将52只SD大鼠随机分为4组:假手术组(n=10)、模型组(n=14)、枳实薤白桂枝汤组(n=14)、加减枳实薤白桂枝汤组(n=14)。大鼠灌胃给药14天后结扎心脏左冠状动脉前降支,按缺血30 min再灌注120 min的方法建立心肌缺血再灌注损伤模型。全自动生化分析仪检测血清乳酸脱氢酶(LDH)、心肌肌钙蛋白I(cTnI)变化;HE染色观察心脏组织病理形态学变化;酶联免疫吸附法检测血浆腺苷酸环化酶(AC)、环磷酸腺苷(cAMP)水平;免疫组织化学染色检测心肌组织AQP1、AQP4的定位表达;实时荧光定量PCR检测心肌组织AQP1、AQP4 mRNA表达;Western blot检测心肌组织蛋白激酶A(PKA)、磷酸化PKA(P-PKA)、cAMP反应原件结合蛋白(CREB)、磷酸化CREB(P-CREB)及AQP1、AQP4的蛋白表达。 结果 与假手术组比较,模型组血清LDH、cTnI及血浆AC、cAMP含量升高(P<0.01),心肌组织P-PKA、P-CREB和AQP1、AQP4 mRNA及蛋白表达升高(P<0.01);与模型组比较,枳实薤白桂枝汤组、加减枳实薤白桂枝汤组血清LDH、cTnI及血浆AC、cAMP含量降低(P<0.01),心肌组织P-PKA、AQP1、AQP4 mRNA及蛋白表达降低(P<0.01);与枳实薤白桂枝汤组比较,加减枳实薤白桂枝汤组血清LDH、cTnI及血浆AC、cAMP含量降低(P<0.01),心肌组织P-PKA、AQP1、AQP4 mRNA及蛋白表达降低(P<0.01)。 结论 加减枳实薤白桂枝汤可以改善缺血再灌注后心肌细胞病理损伤,其作用机制可能与下调AC/cAMP通路,进而抑制AQP1、AQP4 mRNA及蛋白表达有关。

    Abstract:

    Aim To investigate the effect of modified Zhishi Xiebai Guizhi decoction (Zhishi, Xiebai, Sanqi, Renshen, etc.) on the expression of aquaporin (AQP) in myocardial tissue of ischemia-reperfusion rats and its possible mechanism. Methods 52 SD rats were randomly divided into 4 groups:sham operation group (n=10), model group (n=14), Zhishi Xiebai Guizhi decoction group (ZXGD group, n=14) and modified Zhishi Xiebai Guizhi decoction group (mZXGD group, n=14). Left anterior descending coronary artery was ligated after 14 days of intragastric administration, and the myocardial ischemia-reperfusion injury model was established according to the method of ischemia 30 minutes and reperfusion 120 minutes. Changes of serum lactate dehydrogenase (LDH) and cardiac troponin I (cTnI) were detected by automatic biochemical analyzer. Change of cardiac histopathology was observed by HE staining. The levels of plasma adenylate cyclase (AC) and cyclic adenosine monophosphate (cAMP) were detected by enzyme-linked immunosorbent assay. Immunohistochemical staining was used to detect the positioning expressions of AQP1 and AQP4 in myocardial tissue.AQP1 and AQP4 mRNA expressions in myocardial tissue was detected by real-time fluorescence quantitative PCR. Western blot was used to detect protein expressions of protein kinase A (PKA), phosphorylated PKA (P-PKA), cAMP-response element binding protein (CREB), phosphorylated CREB (P-CREB), AQP1 and AQP4 in myocardial tissue. Results Compared with sham operation group, the contents of serum LDH, cTnI and plasma AC, cAMP were increased (P<0.01), and the expressions of P-PKA, P-CREB, AQP1, AQP4 mRNAs and proteins in myocardial tissue were increased (P<0.01), in model group. Compared with model group, the contents of serum LDH, cTnI and plasma AC, cAMP were decreased (P<0.01), and the expressions of P-PKA, AQP1, AQP4 mRNAs and proteins in myocardial tissue were decreased (P<0.01), in ZXGD group and mZXGD group. Compared with ZXGD group, the contents of serum LDH, cTnI and plasma AC, cAMP were decreased (P<0.01), and the expressions of P-PKA, AQP1, AQP4 mRNAs and proteins in myocardial tissue were decreased (P<0.01), in mZXGD group. Conclusion Modified Zhishi Xiebai Guizhi decoction can improve the pathological damage of myocardial cells after ischemia-reperfusion, and its mechanism may be related to down-regulating the AC/cAMP pathway and inhibiting AQP1, AQP4 mRNAs and proteins expression.
